It is keyhole surgery to remove a tumour or enlarged lymph nodes from the retroperitoneum, the space at the back of the abdomen around the kidneys, aorta and vena cava. The surgeon works through small incisions with a magnified 3D view. It suits selected, well-defined tumours; larger tumours usually need open surgery.
Retroperitoneal sarcoma is usually treated by open surgery, removing the tumour in one piece together with any organ it is attached to, because this lowers the chance of the tumour coming back. Robotic removal is considered only for small, selected tumours where complete removal is clearly possible. The plan is made in a tumour board after a CT or MRI scan and, often, a core biopsy.
For suitable tumours, robotic surgery means smaller wounds, less pain and a shorter stay. The risks are those of any major abdominal operation: bleeding from large blood vessels, injury to nearby organs, and conversion to open surgery if the tumour is stuck. For large or invasive tumours, open surgery is the safer choice, not a lesser one.
The robot gives a magnified 3D view and wristed instruments that work well in the deep, narrow space around the kidneys and the major blood vessels. This helps with careful removal of lymph nodes and small tumours. The benefit depends on choosing the right patient for the approach.
After robotic surgery most patients walk within a day and go home in 2 to 4 days; after open surgery the stay is usually 5 to 7 days. Light activity resumes in 2 to 3 weeks and full activity in 4 to 6 weeks, depending on how extensive the operation was.
Ask whether the surgeon routinely treats retroperitoneal tumours, whether your case will be discussed in a tumour board, and whether the hospital has vascular surgery and intensive-care support. Dr Swati Shah, Robotic Cancer Surgeon (Urology & Gynec), has 18 years of experience in cancer surgery and operates at Apollo Hospital, Bhat, Gandhinagar.
The outcome depends mainly on the type and grade of the tumour and on whether it is removed completely with clear margins. Benign tumours are usually cured by complete removal; sarcomas need long-term follow-up with scans because they can come back. Your surgeon will explain the expected outcome once the biopsy and scans have been reviewed.
The cost depends on the size and position of the tumour, whether other organs must be removed, and the length of stay. Apollo Hospital, Bhat supports cashless insurance, and a written estimate is given after the scans have been reviewed.
