| Types of Surgery | Description | Benefits |
|---|---|---|
| Open Surgery | Involves a large incision to remove the tumor | Allows for direct access and removal of large tumors |
| Laparoscopic Surgery | Uses small incisions and a camera to guide the surgery | Less invasive, with smaller incisions and shorter recovery times |
| Robotic Surgery | Keyhole surgery | Less Pain, Less hospital stay, Early recovery |
| Better View (3D, 10x, HD) | Less blood loss, Better nerve preservation | |
| Flexible Instruments | Enhances Precision, Enhances Accuracy, Early functional recovery |

The first operation for a testicular tumour is a radical inguinal orchiectomy: removal of the testis through a small groin incision. It is a short open operation and is not done robotically. Robotic surgery is used later, for selected patients who need a retroperitoneal lymph node dissection (RPLND), the removal of lymph nodes from the back of the abdomen through small keyhole incisions.
For a suitable patient who needs RPLND, the robotic approach usually means smaller wounds, less pain, less blood loss and a shorter hospital stay than the open operation. A nerve-sparing technique aims to protect the nerves that control ejaculation. Not every patient is suitable: large masses left after chemotherapy are often safer to remove by open surgery.
The most widely used platform in India is the da Vinci system made by Intuitive Surgical; a few newer platforms are also in use. For the patient, the surgeon's experience with RPLND and the team's ability to convert to open surgery when needed matter more than the make of the robot.
Dr Swati Shah, Robotic Cancer Surgeon (Urology & Gynec), has 18 years of experience and operates at Apollo Hospital, Bhat, Gandhinagar, with consultations at Shah's Cancer & Robotic Surgery Centre, Gota. Each case is discussed with medical oncology and radiology before surgery, because many testicular cancers are treated with surveillance or chemotherapy alone after the orchiectomy.
Orchiectomy is a short open operation, so the robotic cost question applies only to RPLND. Robotic RPLND has a higher theatre cost, which is partly offset by a shorter stay and a quicker return to work. The final figure depends on the procedure, room category and insurance cover; a written estimate is given after the consultation.
After orchiectomy most men go home the same day or the next day and return to desk work in about a week. After robotic RPLND the usual hospital stay is 2 to 4 days, with light activity in 2 to 3 weeks. Heavy lifting is avoided for about 4 to 6 weeks.
