Breast conservation surgery removes the cancer and keeps the breast. It is carried out by Dr Swapnil Sharma, MBBS, MS, FMAS, DrNB (Surgical Oncology), Consultant Surgical Oncologist, at Apollo Hospital International Ltd, Bhat, Gandhinagar.

Breast conservation suits most early breast cancers. It depends on tumour size against breast size, and on whether the tumour sits in one area.
The question is not how big the tumour is on its own. It is how big the tumour is against the breast it sits in. A 2 cm lump in a large breast leaves a good shape. The same lump in a small breast may not.
Conservation is harder to offer when disease is spread across more than one part of the breast, when the tumour is large against the breast, or when radiotherapy afterwards is not possible. In those cases removing the breast is the safer route, and it is discussed openly.
The operation is done under general anaesthesia. It takes about one to two hours.
If the lump cannot be felt, a wire or a marker is placed in it first, using ultrasound or mammogram guidance. That tells the surgeon exactly where to go. The lump is then removed with a rim of normal tissue around it.
The tissue is X-rayed in theatre to confirm the whole lesion is out. Small clips are left in the cavity so the radiotherapy team can aim at the right place later. The lymph nodes under the arm are checked in the same sitting, usually by sentinel node biopsy.
Where removing the lump would leave a dent, the remaining breast tissue is moved to fill it in the same operation. That is oncoplastic surgery.

Neither column is the right answer on its own. The choice is made with you, once the reports are in.
Most women go home within one or two days. Light work returns in about two weeks. Heavy lifting waits about six weeks.
There is soreness rather than sharp pain, and simple tablets handle it. A soft supportive bra day and night for a few weeks helps a great deal.
Bruising and mild swelling settle over two to three weeks. Shoulder exercises start early and are shown to you before you go home. Keeping the shoulder moving is the single thing that prevents a stiff arm later.
The tissue report comes back in about a week. That report, not the operation alone, decides what treatment follows.
Yes, for most early breast cancers. Breast conservation with radiotherapy and mastectomy give the same long-term survival. Trials have followed women for twenty years to show this.
The lump is taken with a rim of normal tissue around it. How much that is depends on the size of the tumour. The aim is a clear edge with as little tissue loss as can be managed.
The report may show tumour cells close to the cut edge. A second, smaller operation then removes a little more tissue. This is not a setback. It is part of getting the margin right.
There is usually some change in shape. Where a larger piece has to come out, the breast can be reshaped in the same sitting so the result stays even. Dr Swapnil Sharma does this reshaping himself.
Almost always, yes. Radiotherapy to the rest of the breast is what makes conservation as safe as mastectomy. It usually starts a few weeks after surgery, once the wound has settled.
Most women are home within one or two days. Desk work and driving usually return in about two weeks. Heavy lifting and gym work wait about six weeks.
