In short
Large trials in many countries compared keyhole and open surgery for colon cancer. The cancer results were the same. Keyhole patients recovered faster, with less pain.
Robotic surgery is a form of keyhole surgery. It is safe for colon cancer, but no trial has shown it controls the cancer better than standard keyhole surgery.
Open surgery is still the safer choice for some people. These include very bulky tumours, a blocked or burst bowel, and a badly scarred abdomen.
When keyhole colon surgery began, nobody doubted it could be done. The worry was whether it removed the cancer as well as open surgery.
Two fears drove the trials. The first was that cancer cells might settle in the small cuts where the tubes enter. The second was that, without a hand inside the belly, the surgeon might remove too little bowel or too few lymph nodes.
So the trials did not only measure how fast people went home. They followed patients for years and asked one question. Does the cancer come back more often after keyhole surgery? Those long follow-ups are what settled the matter.
Each trial below randomly placed patients into keyhole or open surgery. They were run in different health systems, by different teams.
| Trial | Where | What it found |
|---|---|---|
| Barcelona (Lacy) | Spain | The first trial to suggest keyhole surgery was not worse for the cancer. |
| COST | USA and Canada | Cancer return and survival were the same. Recovery was faster. |
| COLOR | Europe | The same cancer results across many hospitals. The operation took longer. |
| CLASICC | UK | The same survival. Many operations changed to open early in the trial. |
| ALCCaS | Australia and New Zealand | The same long-term results. The recovery gain was confirmed. |
| JCOG0404 | Japan | The same results even with a full lymph node clearance. |
The cancer side of the operation matched in every trial. The difference was in recovery: less pain, earlier eating, a shorter stay and fewer wound problems.
| Question | Keyhole | Open |
|---|---|---|
| Does the cancer come back more often? | No. The same. | The same. |
| Length of life after surgery? | The same. | The same. |
| Clear edges and lymph nodes removed? | The same. | The same. |
| Pain after surgery? | Less. | More. |
| Eating again? | Sooner. | Later. |
| Hospital stay? | Shorter. | Longer. |
| Wound infection and hernia? | Fewer. | More. |
| Time in theatre? | Longer. | Shorter. |
The fear about cancer growing in the small cuts was not borne out. It was rare, and no more common than in an open wound. Good habits prevent it in any approach. These include tying the main vessels before the tumour is handled, and protecting the wound the bowel comes out through.
The trials studied planned operations in people who suited either approach. Some groups were left out or barely included. For them, the trial result cannot simply be borrowed.
In these situations open surgery is often the safer plan. Keyhole surgery is still possible in some, in experienced hands, with a low threshold to change to open.
Sometimes a keyhole operation is changed to open partway through. Done early and calmly, this is sound judgement, not a failure.
Common reasons are a bulky tumour, heavy body weight, dense scar tissue from old operations, or anatomy that is hard to see. Changing early, before anything goes wrong, works out better than changing late after a problem. The plan to change, if needed, is discussed with you before surgery.
For colon cancer, the robot changes how the surgeon works, not what is removed. The cancer operation is the same in all three approaches.
The robot gives a steady 3D view and wristed instruments. This can help with joining the bowel inside the belly, with clearing the lymph nodes around the main vessels, and in heavier patients. It costs more than standard keyhole surgery.
No large trial has shown longer survival after robotic colon surgery than after keyhole colon surgery. The robot’s clearest advantage is lower down, in the narrow pelvis, for rectal cancer. That is a separate question with its own evidence, set out on the robotic rectal cancer surgery page.
Which robotic colon operation suits which tumour site is explained on the robotic colon cancer surgery by tumour site page.
The trials took care to use surgeons who were already experienced in keyhole surgery. The equal results depend on that experience. A sound cancer operation matters more than the name of the method.
Recovery also depends on the care around the operation. Early walking, early eating and a plan to avoid long tubes and drains help with any approach. Keyhole surgery gives its full benefit only inside such a recovery plan.
Yes, for planned colon cancer surgery in experienced hands. Large trials found the same cancer return and survival with both.
No trial has shown that for the colon. The robot helps the surgeon's view and movement. It removes the same bowel and nodes.
This was an early fear. Trials showed it is rare, and no more common than in an open wound.
For tumours grown into nearby organs, a blocked or burst bowel, a badly scarred abdomen, and some frail patients.
No. Rectal cancer has its own trials and its own questions. This page is about the colon only.
