Real-World Maintenance Treatment Patterns in Advanced Ovarian Cancer Since 2020

For women with advanced ovarian cancer, treatment usually begins with chemotherapy. After chemotherapy, some patients receive “maintenance therapy” to help keep the cancer from coming back. Since 2020, more options have become available, including PARP inhibitors and bevacizumab, but not all patients receive these treatments.

A new US study looked at nearly 600 women with stage III or IV ovarian cancer who started chemotherapy between 2020 and 2023. The researchers wanted to see how many patients received maintenance therapy and what treatments were used.

They found that fewer than half of women (48%) received any type of maintenance therapy after chemotherapy. The most common options were:

PARP inhibitor pills alone (23%)
Bevacizumab (a targeted drug given by IV) alone (15%)
Combination of PARP inhibitor plus bevacizumab (8%)

Genetic testing played an important role. Women with a BRCA gene mutation were most likely to receive PARP inhibitors, either alone or in combination with bevacizumab. Nearly half of BRCA-positive women received a PARP inhibitor, compared with only about one in five women without the mutation. On the other hand, women without BRCA mutations were more likely to receive bevacizumab alone.

The study highlights that even though effective maintenance therapies are available, many women do not receive them. The reasons may include side effects, cost, access to genetic testing, or differences in how doctors make treatment decisions.

For women with advanced ovarian cancer, it is important to talk with their doctors about genetic testing and whether maintenance therapy may be right for them. These treatments can help delay cancer from coming back and may improve long-term outcomes.
