Preoperative Radiotherapy Plus Surgery Versus Surgery Alone for Patients with Retroperitoneal Sarcoma A Systematic Review and Meta-analysis.DOI 10.1002jso.27845

Retroperitoneal sarcoma is a rare cancer that grows deep inside the abdomen. Surgery is the main treatment, but doctors are still studying whether radiotherapy before surgery can improve outcomes.A recent study reviewed several clinical studies involving more than 1,200 patients to compare two treatment approaches: surgery alone and radiotherapy followed by surgery.

The results showed that adding radiotherapy did not significantly increase overall survival for all patients with retroperitoneal sarcoma.

Patients lived about the same length of time whether they received radiotherapy before surgery or surgery alone.

Even so, radiotherapy provided an important benefit in some cases.

Patients who received radiotherapy before surgery had better local tumor control, meaning the cancer was less likely to return in the same area.

This advantage was most noticeable in patients with certain tumor types, especially low- or intermediate-grade liposarcomas, which are common forms of retroperitoneal sarcoma.

The study also found that serious postoperative complications were similar between the two groups. This suggests that adding radiotherapy before surgery did not significantly increase surgical risk. These findings suggest that preoperative radiotherapy may be useful for selected patients, depending on the tumor type and characteristics.

Doctors may use a personalized treatment approach, deciding whether to add radiotherapy based on the specific subtype of sarcoma and the patient’s overall condition.
