Immunotherapy Side Effects May Be Linked to Better Survival in Liver Cancer

Immunotherapy is becoming an important treatment for people with advanced liver cancer (hepatocellular carcinoma). One combination therapy called STRIDE—which includes tremelimumab and durvalumab—has already shown better survival compared to older treatments.This new study looked at whether immune-related side effects might be linked to how well the treatment works. These side effects happen when the immune system becomes more active and starts affecting normal tissues while attacking cancer.
Researchers studied patients from the large HIMALAYA clinical trial. About 36% of patients receiving the STRIDE treatment experienced immune-related side effects, while fewer patients taking durvalumab alone had these effects. Most of the side effects were mild and manageable, and they usually appeared within the first three months of treatment.
Interestingly, patients who developed these immune-related effects while receiving STRIDE tended to live longer than those who did not. After three years, about 36% of patients with these side effects were still alive, compared with about 28% of those without them.
The most common side effects involved hormone-related problems, which doctors can usually treat with medication.
These findings suggest that when the immune system becomes active enough to cause mild side effects, it may also be working harder to fight the cancer. The results continue to support STRIDE as an effective treatment option for patients with unresectable liver cancer.
