HIPEC is a heated chemotherapy treatment given directly into the belly after surgery to remove cancer. It’s used for cancers that have spread in the abdominal area. Keeping the right temperature during this treatment may be more important than we thought.

This study looked at over 750 patients who had surgery and HIPEC. It found that people lived longer when the heated chemotherapy was kept at or above 40.5°C for most of the treatment time—at least 70% of the 90-minute session. These patients were called the PASS group. They lived a median of 149 months compared to 80 months for those where the heat wasn’t kept as steady (FAIL group).

The benefit was strongest when using mitomycin-C or melphalan, two common HIPEC drugs. The temperature didn’t seem to matter as much for platinum-based drugs. Patients with complete tumor removal and less spread of disease also did better when the right heat levels were maintained.

This research shows that staying within the best temperature range during HIPEC can help people live longer. It’s not just the drug, but how it’s delivered. Patients planning to get HIPEC can ask their care team how temperature is managed during the procedure.
