How Often Should Men Repeat the PSA Test? New Review Finds Gaps in Current Guidelines

The PSA blood test helps detect prostate cancer early, but how often men should repeat the test is still unclear. A new review looked at international medical guidelines to find out what experts recommend and how strong the evidence really is.

Researchers analyzed 11 major guidelines published between 2013 and 2024. Most of them advised repeating the PSA test every 2 to 4 years for men aged 50 and older who do not have symptoms. If a man’s PSA level is very low—below 1 ng/mL—the next test could safely be delayed up to 10 years.

But the study found that many of these recommendations are not strongly supported by research. Only a small number of studies directly tested how often PSA should be repeated. In some cases, the advice in the guidelines did not fully match the evidence they cited.

This means doctors and patients need to make decisions about PSA retesting based on individual health, risk factors, and personal preferences. For example, men with higher PSA levels or a family history of prostate cancer may need closer monitoring, while those with low levels may not need testing as often.

Until more research provides clear answers, regular discussions between men and their doctors remain the best way to decide when to repeat PSA testing. The goal is to balance early detection with avoiding unnecessary tests and anxiety.
