A Safer, More Accurate Way to Test for Prostate Cancer

When doctors suspect prostate cancer, they often need to take a biopsy—a small sample of prostate tissue—to confirm the diagnosis. There are two common ways to do this: through the rectum (transrectal) or through the skin between the scrotum and anus (transperineal). This study compared both methods using MRI to guide the biopsy.

Ninety men took part. Some were being monitored for low-risk prostate cancer, while others had suspicious MRI results or previous negative biopsies. Half had a standard transrectal biopsy using local anesthesia. The other half had an MRI-guided transperineal biopsy under general anesthesia.

The results showed that both methods were safe, with few side effects. But the transperineal approach found more cancers—especially the ones that mattered. Nearly 28% of men in this group had clinically significant prostate cancer, compared to just 8% in the transrectal group.

Even though both groups reported similar levels of comfort and quality of life, more men in the transperineal group had changes to their treatment plan. This suggests that the more accurate biopsy helped doctors make better decisions.

This study shows that MRI-guided transperineal biopsy is a safe and more effective option for detecting prostate cancer, especially in men who have had a previous negative biopsy or are being closely watched.
