Development and Validation of an Artificial Intelligence Digital Pathology Biomarker to Predict Benefit of Long-Term Hormonal Therapy DOI- 10.1200JCO.24.00365

Men with high-risk prostate cancer often receive hormone therapy for up to two years after radiation. This can slow cancer, but it also brings side effects like hot flashes, fatigue, and bone loss. A new study shows that artificial intelligence (AI) may help some men safely skip the extra months of treatment.

Researchers used digital scans of prostate biopsy tissue and clinical data to train an AI tool. It learned to predict which men benefit most from long-term hormone therapy. The tool was tested in a group of over 1,100 men followed for more than 17 years.

The AI identified two groups. About two-thirds of the men were “biomarker-positive.” These men clearly benefited from long-term hormone therapy. Their risk of cancer spreading to other parts of the body dropped from 26% to 17%.

The other one-third were “biomarker-negative.” For them, long-term therapy didn’t help. These men could avoid extra treatment without raising their risk.

This new AI tool could lead to more personalized care. It may help doctors decide who really needs long-term hormone therapy and who doesn’t—saving some men from unnecessary side effects.
